1) My involvement/participation in 360 Self-Defence self-defence training is voluntary in that I am acting of my own free will and volition. I understand that I may cease my instruction at any time and will receive no refund for any monies paid.

2) I understand that there is a risk of danger, bodily harm, injury (including but not limited to cuts, scratches, bruises, muscle strains and tears, broken bones) emotional stress or death as a result of my participation in the afore mentioned training provided by 360 Self-Defence and Wendy Kipka. I acknowledge there is a potential for risks and dangers to myself and others that may not be obvious or reasonably foreseeable at this time. I am learning and participating in instruction provided by 360 Self-Defence and Wendy Kipka with full knowledge and understanding of the potential of serious danger in the activities and hazards involved in the training being provided by 360 Self-Defence and Wendy Kipka. I hereby consent and agree to any physical contacts to me by my instructor, fellow students or others in furtherance of training, practice or demonstration. I hereby consent and agree to completely accept alone any liability whatsoever arising out of any activity relating to instruction, demonstration/competition, or practice (on or off premises). I acknowledge, confirm and verify the foregoing.

3) I do not have any medical ailments, physical limitations, or mental disabilities that will affect my ability to participate in 360 Self-Defence and Wendy Kipka’s self-defence instruction and have disclosed any injuries or medical conditions that could injure me or others. I acknowledge that 360 Self-Defence and Wendy Kipka to recommend that I seek the approval of a physician before engaging in any physical activity such as the kind of activity being provided by 360 Self-Defence and Wendy Kipka. If the condition of my health changes during the time of existence of this agreement I will promptly advise 360 Self-Defence and Wendy Kipka. I will cease any activity during my training that I deem to be injurious or potentially injuries to my health and well-being or the well-being and health of others. I acknowledge that I alone am responsible for any existing health ailments and future ones whether acquired through training provided by 360 Self-Defence and Wendy Kipka or elsewhere.

4) I understand that 360 Self-Defence and Wendy Kipka cannot guarantee my safety or the safety of others through the use, or misuse, of the techniques taught by 360 Self-Defence and Wendy Kipka. 360 Self-Defence and Wendy Kipka are not liable for any outcome derived from my use, attempted use, failure to use, or misuse of these techniques during my instruction/training or outside of my instruction/training. Any activity which I deem to be unsafe to my personal health or others, or potentially unsafe, I will notify 360 Self-Defence and Wendy Kipka and cease to participate in the activity.

5) 360 Self-Defence Wendy Kipka undertakes no direct legal or financial responsibility for my personal safety or well-being.

6) I assume all risks, including but not limited to those outlined above in section 1 through 6 of this agreement.

7) I forever release 360 Self-Defence, Wendy Kipka and any instructors, employees, volunteers and all related participants, from any and all claims and causes of action that I now, or may have in the future, for any type of personal injury, medical ailment, negligence, psychological injury, property damage or wrongful death occurring to me or any person arising out of, or connected to, training provided by 360 Self-Defence and Wendy Kipka and affiliates.

8) I am 100 per cent liable for all expenses incurred as a result of any injury or illness, to myself or others, or property damage incurred during, or connected to, my participation in training provided by 360 Self-Defence and Wendy Kipka.

9) In the event that any one or more of the provisions of this agreement shall be held to be invalid, illegal, and unenforceable or in conflict with the law, the remaining portions will not be invalidated, and shall remain in full force and effect.

10) I consent to the use of my likeness in images or video for promotional purposes on the internet or other mediums. If no consent is given I am responsible for advising instructors and removing myself from photo or other recording events.

11) I consent to Wendy Kipka and 360 Self-Defence reserving the right to remove me from class and cancel any services with no refund should my conduct or behavior be deemed to be unsafe, dangerous or disruptive to other students and or the instructors. I agree, and am aware, that prices and class schedules may change at any time.

12) This is a legally binding contract, but it is not meant to pronounce any claims or defenses that are legally prohibited.
